Taipei City Yangming Home for the Disabled

Social Work Section

Goals

We administer the application service and individual case management for Taipei mental disabilities citizens and their families. We also provide group visit service and coordinate social resources. Furthermore, we open for colleges students to apply for training supervision.

Overall, we do our best to enrich and promote our service recipients’ quality of life in order to offer a friendly, barrier-free environment.

Services:

A. Individual case management service — Establish familiarity with service recipients and assist them in recognizing the support systems and service network.

B. Social resources coordination and management — to wildly recruit social resources and volunteers, and to manage donations.

C. Promote the visit service — include visitor accommodation, transact outward circumstance, and edit publications.

D. Organizing large activities — to organize leisure activities along with festival celebration in order to enrich the service recipients' lives.

E. Internship assistance and counseling — internship applications are open to college students majoring in social work in hopes of educating new blood and nurturing their professional expertise to carry on future social work-related projects.

F. Safeguarding the rights and interests of the Institute’s service recipients — cell group meetings for promoting the rights and interests of the Institute’s service recipients are regularly held; campaigns for promoting initiatives concerning the rights and interests of service recipients are organized periodically. The Institute also provides medical insurance services and contract replacements or renewals for service recipients.

G. Communication channels established between the Home and parents — PTA meetings, parents’ seminars and parents’ book clubs are organized regularly in hopes of building an effective communication conduit between parents and the Home.

Health Services Section

Goals

The Health Services Section consist of one section chief, two nursing chiefs, twelve registered nurses, one nutritionist, eight physical therapists, five occupational therapists, and two pharmacists, devoted to service recipients’ health management and health improvement. The team is actively seeking service programs and policies to effectively slow down service recipients’ aging process, providing professional cares and leisure activities that are beneficial to service recipients’ quality of life, thus alleviating their burdens and medical expenses. The various services provided by this section are not only based on the “longitude” of nursing team with prevention, nutrition, medication and treatment, but are also based on the “latitude” of rehabilitation team with health promotion, reablement, assistive technology and counseling, to create one medical service network, two task forces, and three developmental directives to create a healthful, safe and dignified home for the disabled. Under the leadership of current superintendent, this section introduces many pioneering, comprehensive and sustainable measures.

Services:

A. Establishing medical network – The Home is medically supported by Yang-Ming Branch, Taipei City Hospital. Outpatient services available include: Family Medicine Pediatrics, Dentistry, Rehabilitation, Neurology, Dermatology, and Psychiatry. Service recipients suffering from acute diseases or accidents are sent to the Yang-Ming Branch for treatment. The hospital also provides on-call outpatients and emergency services, so that service recipients receive the best possible care to ensure their health. Yang-Ming Branch also provides preferential discounts in medical expenses according to individual needs.

B. Two task forces – the Section is actively involved in “Service Orientation Project Task Force” and “Interdepartmental Task Force” to discuss and research issues on aging among service recipients, individualized support assessment, family service demands, vocational counseling and evaluation, and promotion of rights.

C. Three developmental directives – including disease prevention, health promotion and dignified living. Each directive consists of many branch projects. Disease prevention includes annual health checkups, infection control, and outpatient emergency services. Health promotion encompasses the caring of overweight service recipients/underweight service recipients, dental hygiene and care. Dignified living constitutes physical fitness programs, communicative skills promotion (including the use of easy read), vocational and potential development projects.

Education and Training Section

Goals

Based on the degrees of disabilities of our service recipients, individual education and training plans for service recipients are designed to provide life care, residential rehabilitation and health care with the goal of self-daily life handling. Meanwhile, leisure activities, social adaptation activities and diverse learning activities are designed to enhance the quality of life.

Services:

A. Living assistance – assistance and counseling are provided to service recipients in dining, toileting, bathing, bedtime and rehabilitation according to their level of self-care abilities, to enhance their self-sustainable living abilities.

B. skill enlightenment services – Service recipients would be given expertise training of different fields to enhance their cognitive and self-expression abilities, paving the way for their future in adapting to various leisure and recreational skills.

C. Nursery services – Service recipients’ physical capabilities are evaluated for outlining individualized service programs to include positioning, standing, passive exercise, feeding, sensory stimulation and other trainings to improve their self-care skills.

D. Community adaptation – to encourage service recipients’ social participation and be an active part of the community. Community adaptation courses are organized periodically as residents are taken to various community settings to improve their adaptability: shopping, taking buses, interpersonal contact, etc.

E. Vocational counseling – service recipients are given vocational training courses based on their level of skills; they are taught to clean, fold laundry, handicraft lessons, etc, to better develop their vocational potentials.

Vocational Development Section

Goals

In order to reach the goal of vocational skill training and provide the individual service programs, we carry on pre-training assessment. According to our service recipients' abilities, we design agriculture, dining, baking, and handicrafts programs. Besides vocational training, we also offer various kinds of recreational activities and cognition curriculum to provide an all-directions service.
