The applicant go to examining units to apply for qualification examining after preparing all the related documents, and then send them (application form, front and back photocopy of identity card, 2-inch color photograph, and the lottery dealer’s original of the first sheet of first stage qualification examining form) by registered mail by themselves to the Taipei Post 51-120 mail “China Trust Commercial Bank Lottery Center”
